Our PersonFirst® Approach

At Holbrook, our overriding belief is that every resident is a unique person, has a story to tell, and should be encouraged to participate in guiding their own care.

Our approach to care flows out of the relationships that we build with our residents. Because we take the time to get to know deeply each of our residents, we are better informed and guided as to how best to manage their care.

We call this our PersonFirst Approach, and it has three components: My Plan, Life Stories, and All Hands.

Accredited by both the Commission on Accreditation of Rehabilitation Facilities (CARF) and the Continuing Care Accreditation Commission (CCAC), Holbrook was awarded Exemplary status for our innovative Life Stories program.
